What research shows about Ritalin, Concerta and methylphenidate in adults
Methylphenidate is the second large group of ADHD stimulants next to the amphetamines. Ritalin is the immediate-release brand, and Concerta is an osmotic-release extended-release brand, so both contain the same active ingredient with a different duration.
How well does methylphenidate work?
A Cochrane review of 16 trials with 3,799 adults (Boesen, 2022) found lower self-rated ADHD symptoms with methylphenidate than with placebo (SMD -0.37). The authors rated the evidence as very low quality, and methylphenidate was not better than placebo for missed days at work.
In a 6-week trial, 68 percent of adults on methylphenidate had at least a 30 percent drop in symptoms, against 17 percent on placebo. In a 24-week trial with 359 adults, extended-release methylphenidate at 0.55 mg per kg body weight lifted this rate to 61 percent, against 42 percent on placebo.
A network meta-analysis of 51 trials with 8,131 adults (Cortese, Lancet Psychiatry 2018) found amphetamines slightly ahead of methylphenidate after about 12 weeks (SMD -0.29), with more dropouts from side effects on amphetamines. An earlier meta-analysis of 11 trials with 1,991 adults (Mészáros 2009) reported a Cohen’s d of 0.67 for stimulants overall.
How long does each form last?
Short-acting methylphenidate lasts about 3 to 5 hours, intermediate forms 4 to 8 hours, and long-acting forms 8 hours up to 12 hours or more. Some patients combine a long-acting morning dose with a short-acting dose later in the day, so the evening is covered.
What does ADHD medication change in daily life?
Among 2,319,450 patients in a US insurance database, the collision risk was 38 percent lower in men (OR 0.62) and 42 percent lower in women (OR 0.58) in the months on ADHD medication (Chang, JAMA Psychiatry 2017).
What are the side effects and risks?
About 10 percent of adults stop stimulants because of side effects across studies. Blood pressure rises by about 3 to 5 mmHg and heart rate by about 5 beats per minute on average. Priapism (a prolonged painful erection) is a rare effect of methylphenidate.
In pregnancy, the decision is shared between patient and psychiatrist. One study found a relative risk of 1.28 (95% CI 1.00 to 1.64) for cardiac malformations after first-trimester exposure, while a meta-analysis found an odds ratio of 1.1.
Atomoxetine is the main non-stimulant. In 12 trials with 3,375 adults it lowered symptoms (SMD -0.33), and 17.2 percent stopped it within 6 months, against 5.6 percent on placebo.
How does treatment usually run?
Treatment starts at a low dose, and the dose rises step by step, about weekly. Check-ups take place about once a month until the dose is stable, with blood pressure and pulse at each visit. An ECG is added with heart disease or heart symptoms.
