In short

  • A child ADHD evaluation can start from age 4 when symptoms of inattention, hyperactivity or impulsivity appear.
  • Children under 17 need at least 6 symptoms of inattention or 6 of hyperactivity and impulsivity, present often, for 6 months or more, in more than one setting, with onset before age 12.
  • Parents and teachers complete ADHD rating scales, which show more than 90 percent sensitivity and specificity in suitable groups.
  • The full evaluation can take several visits.
  • Methylphenidate lowered teacher-rated symptoms in 21 trials with 1,728 children (SMD -0.74).
  • Teens are interviewed privately, because they may share what parents and teachers do not know.

How does a child ADHD assessment work?

A child ADHD assessment collects the medical, developmental, school and family history, interviews parents and child and reviews information from school. Rating scales for parents and teachers show whether symptoms occur in more than one setting.

No single test proves ADHD. The psychiatrist also looks for other explanations, such as learning problems, sleep problems or anxiety, and for conditions that occur together with ADHD.

What research shows about ADHD in children and teens

The numbers below describe when ADHD shows, how it is assessed and how treatment performs in children.

When do the symptoms appear?

Hyperactive and impulsive symptoms usually show by age 4, increase over the next 3 to 4 years and peak at age 7 to 8. Inattention is often not apparent until age 8 to 9. Hyperactivity then declines, and impulsivity and inattention often persist.

How reliable are the tools?

ADHD-specific rating scales show a sensitivity and specificity above 90 percent when used in a suitable population. Broad behaviour scales fall below 86 percent and serve to find coexisting conditions. A 2013 meta-analysis of 9 studies with 1,253 children with ADHD and 517 without found that EEG patterns cannot diagnose ADHD reliably, so the diagnosis rests on history, observation and rating scales.

What is different in teens?

Teens may underreport symptoms, so information from at least two teachers or other adults helps. In a follow-up of 239 young people without childhood ADHD, a late-onset diagnosis was excluded in about 95 percent of those who screened positive, mostly through missing impairment, missing confirmation across settings or another explanation such as substance use.

How well does medication work?

A meta-analysis of 21 trials with 1,728 children found lower teacher-rated symptoms with methylphenidate than with placebo (SMD -0.74, 95% CI -0.88 to -0.61). Five placebo-controlled trials with 1,044 children showed an SMD of -1.16 for amphetamines at about 12 weeks. A network meta-analysis of 81 trials with 10,068 children (Cortese, Lancet Psychiatry 2018) placed amphetamines slightly ahead of methylphenidate (SMD -0.32), with more dropouts from side effects.

In pre-school children aged 3 to 5.5 years, 6 trials with 651 children showed an SMD of 0.62 for methylphenidate. In the PATS trial, 11 percent stopped the medicine because of side effects, and moderate to severe side effects occurred in 30 percent.

What are the side effects?

Common effects are lower appetite, sleep problems, jitteriness and mood swings. Weight, height, blood pressure and pulse are measured at least once per month during dose finding. In a trial with a 16-year follow-up, adult height was 4 cm lower with consistent stimulant use than with negligible use, and growth effects tend to fade over time.

Parents give feedback on behaviour, and teachers give feedback on school. The first months are used to find the dose, with check-ups every 3 to 6 months afterwards.

Medication for children in the UAE

Some ADHD medicines are controlled in the UAE. Methylphenidate and the amphetamines are in Psychotropic Schedule II of the Emirates Drug Establishment list, so a licensed psychiatrist prescribes them only where UAE rules allow. Atomoxetine and guanfacine are not on that list.

Frequently asked questions about child ADHD assessment in Dubai

Short answers about the assessment, the criteria and treatment.

An evaluation can start from age 4 when symptoms of inattention, hyperactivity or impulsivity are present. The criteria are the same for ages 4 to 17.

Children under 17 need at least 6 symptoms of inattention or 6 of hyperactivity and impulsivity. The symptoms occur often, last at least 6 months, appear in more than one setting, began before age 12 and impair daily life.

No single test proves ADHD. The diagnosis rests on history, observation and rating scales completed by parents and teachers, and a 2013 meta-analysis found that EEG patterns cannot diagnose ADHD reliably.

The full evaluation can take several visits, because information from home and school needs to be collected.

Not always. Medication is one option next to behavioural support. In a meta-analysis of 21 trials with 1,728 children, methylphenidate lowered teacher-rated symptoms (SMD -0.74), and the psychiatrist discusses benefits and side effects with you first.

Lower appetite, sleep problems, jitteriness and mood swings are common. Weight, height, blood pressure and pulse are measured at least once per month during dose finding.
