What research shows about Vyvanse and ADHD stimulants in adults
Lisdexamfetamine belongs to the amphetamine group of ADHD stimulants. Large comparisons pool the amphetamines as one group, so the adult numbers below describe the group, with one trial in young patients that names lisdexamfetamine.
How well do amphetamines like Vyvanse work?
A network meta-analysis of 51 trials with 8,131 adults (Cortese, Lancet Psychiatry 2018) compared ADHD medicines after about 12 weeks. Amphetamines reduced symptoms slightly more than methylphenidate (SMD -0.29, 95% CI -0.54 to -0.05).
An earlier meta-analysis of 11 trials with 1,991 adults (Mészáros 2009) found a Cohen’s d of 0.67 for stimulants and 0.59 for non-stimulants. A value around 0.5 counts as a medium effect.
In children and teens, a trial with 317 participants aged 6 to 17 compared lisdexamfetamine, osmotic-release methylphenidate and placebo over seven weeks. Parent-rated achievement rose by 10.2 points with lisdexamfetamine and 7.3 points with methylphenidate against placebo (100-point scale). The trial did not test the two drugs against each other, so the gap between them is not proven.
Why is lisdexamfetamine called a prodrug?
Lisdexamfetamine is a prodrug of dextroamphetamine, which means the body activates it only after it is swallowed and digested. The design aims to discourage misuse, although any stimulant can be misused. In a US survey, 8.9 percent of people prescribed stimulants sold, traded or gave away their medication, which is why the psychiatrist asks about storage and household members.
What does ADHD medication change in daily life?
Among 2,319,450 patients in a US insurance database, the collision risk was 38 percent lower in men (OR 0.62) and 42 percent lower in women (OR 0.58) in the months on ADHD medication (Chang, JAMA Psychiatry 2017).
In a Swedish registry study of about 149,000 people, medication was linked to lower all-cause mortality (HR 0.79) and lower mortality from unnatural causes (HR 0.75) (Li, JAMA 2024).
What are the side effects and risks?
In the network meta-analysis, more adults stopped amphetamines because of side effects than methylphenidate. About 10 percent of adults stop stimulants because of side effects across studies. Blood pressure rises by about 3 to 5 mmHg and heart rate by about 5 beats per minute on average.
Cardiovascular data are mixed. A case-control study of more than 278,000 people found an adjusted odds ratio of 1.09 at 1 to 2 years and 1.23 beyond 5 years of use. A meta-analysis in young and middle-aged adults found a pooled RR of 1.04 (95% CI 0.43 to 2.48), and the wide interval leaves a small risk open.
How does treatment usually run?
Treatment starts at a low dose, and the dose rises step by step, about weekly. Check-ups take place about once a month until the dose is stable, with blood pressure and pulse measured at each visit. An ECG is added with heart disease or heart symptoms. The combination of CBT and medication is preferred: in a trial of 88 adults, 12 group CBT sessions with or without a stimulant improved symptoms at the end of treatment and at 3 months.
